Once you make $100,000.00 you don't pay payroll taxes on the rest of the money. The increasing costs of file keeping, code management and paperwork required by these new HMO's costs the doctors and hospitals a fortune in just these things alone. When I was a kid, most people who worked had Blue Cross blue Shield, and the doctors didn't have to do anything except turn in a bill and get the money the were owed. I didn't have to have a job to buy insurance. I paid $150 a month for full Blue Cross coverage Health care was provided to everyone at one time before HNO’s took over.
