Make it a social call. Say that you come just to say hello, visit your old friends, and let them know how the surgery went. Tell them that you are in perfect condition now, you can get back to work, and you are actually looking for a job. Chances are, he/she will be the one to offer you your job back instead of you having to ask for it. Good luck! Find some courage and do it. I know it can be intimidating, but chances are you left on better terms then you even remembered. I left a job on good terms, but was intimidated to ask to come back six months later. I came back and everyone was happy to see me again.
